Notice of Infringement – Claim

  4. Contact information of the complaining party, including name, address, email, and phone number.
  5. A statement confirming the unauthorized use of the material and the accuracy of the notification.
  6. An acknowledgment that the complaining party is acting on behalf of the copyright owner under penalty of perjury.

Counter Notification – Restoration of Material
